Welcome to the documents page. This page contains documents that new patients must fill out prior to their first functional medicine appointment. There are four documents: introductory information, consent forms, the health questionnaire, and assessment forms. It is important that you read the consent forms, and agree to our fees before you make an appointment. We have only a few spots available for functional medicine visits, and we want to make sure that you do not cancel your appointment at the last minute because of a misunderstanding about our charges.
